AGREE TO PERFORM IN THE NEW AUSSIETWERK VIDEOCLIP - 'From Dusk Till Down 2015'

I do understand that ‘Twerking’ is a type of dancing in which an individual dances to music in a sexually provocative manner involving thrusting hip movements and a low squatting stance and it’s my choice to learn this type of dancing on AussieTwerk classes/rehearsals before the videoclip ‘From Dusk Till Down’ shoot. I do understand that the content of the AussieTwerk videoclip ‘21 From Dusk Till Dawn 2015’ will have a sexually provocative manner (Links on the example Twerk videos are below). I do understand that during the videoclip I shall be performing a Twerk routine in the sexually provocative manner involving thrusting hip movements and a low squatting stance /different shaking movements and it’s my choice to do so. *

I do understand that final AussieTwerk videoclip 'From Dusk Till Down 2015' will be uploaded on such public forums as YouTube & Vimeo *

I agree to attend at least 11 out of 12 rehearsals/classes before the shoot of the AussieTwerk Videoclip 'From Dusk Till Down 2015'. *

If I am not able to attend 2 rehearsals/classes I do agree to take a private lesson with instructor to polish the choreo before the shoot. The cost of one hour private rehearsal/class with instructor will be $75 *

I agree to purchase Costume (Total of $50) by AussieTwerk to perform in it in the AussieTwerk 'From Dusk Till Down 2015' videoclip. *

You are advised to consult your physician before beginning this or any dance/fitness program/course of rehearsals. Do not attempt to perform any movement shown or described on AussieTwerk classes, unless you are certain of your ability to implement it without incurring strain or injury. The owner of AussieTwerk, instructor disclaim any liability for losses or injures sustained in connection with any advice, activity or demonstration on AussieTwerk classes, courses and workshops *

hereby consent to be photographed, recorded and/or filmed by the AussieTwerk (Producer) for the dance video clip “From Dusk Till Down 2015” on the following terms: ● The Producer will own any and all rights in the photography, recording or filming. ● I hereby consent to for the Producer and/or its representatives, agents and related entities to use any photographs, films, videos, digital recordings or audio recordings of my appearance for promotional, commercial, marketing or any other purpose whatsoever, whether or not accompanied by any narration or dialogue, by any present or future media or means known or unknown for this project. ● I waive all personal rights and objections to, including the right to inspect, any use of my appearance by the Producer in connection with the project. ● I understand that the Producer has relied upon my consent in allowing my participation in the Project. ● I understand that compensation (if any) has been agreed in writing in consideration for the above consent. I will not receive any further compensation in the future unless otherwise agreed in writing thus far. ● I shall not disclose to any third party, individual, corporation or other entity unless otherwise indicated in writing by the AussieTwerk, all information of any nature whatsoever described or otherwise made available, orally or in writing, about or relating to the AussieTwerk, it’s projects or it’s clients. *

DEFINITIONS:● (a) Appearance means my name, image, physical likeness, voice, any and all attributes of my personality and/or performance; ● (b) Producer means AussieTwerk and any successors, assigns or licensees; or ● (c) Project means the music video clip on the AussieTwerk choreography “From Dusk Till Down” created by the Producer; ● (d) Use means lawful use and includes the exclusive and perpetual right of the Producer in any jurisdiction worldwide to:(i) use, copy, adapt and/or exploit; ● (ii) mix, edit, duplicate, or reuse, or create derivative work; (iii) assign, license or sublicense; and ● (iv) publicise and/or market; ● (v) exhibitand/orperformand/or distribute by any present or future media,for profit or ● otherwise, and for commercial or noncommercial use. *

Paper videoclip release form will be given to me on AussieTwerk first videoclip rehearsal and I agree to sign it and return to AussieTwerk Producer/Instructor within one week after first rehearsal. *

I'm going to have fun and enjoy every single AussieTwerk rehearsal/class and videoclip shoot;) *
